Enrollment fee to join is $10 per month for individuals and $15 per month for a family. Initial membership period is required to be 6 months ( So individuals pay $60 to join for 6 months and families pay $90 for 6 months). Renewal periods are for 12 months each year (So payments are $120 for individual and $180 for family for annual renewals). Zakat eligible individuals join for free ( when annual income is less than $15,000 ).

Non urgent outpatient medical services only are provided through this program (for example ongoing care for diabetes, high blood pressure, thyroid deficiency, arthritis, stomach acidity etc).

CCN does not pay for any of your healthcare expenses. CCN simply provides you with access to reduced cost, non-urgent health care services. The membership fee is to cover the administrative costs of maintaining the network and its services.

If you do not have separate hospitalization coverage, we recommend you go to the nearest hospital ER and request a Charity Care form to fill and submit. Please note that your CCN physician is not expected to provide care for you in emergencies.
If you use the providers in the network you will pay the discounted fee negotiated by CCN to obtain substantial savings. If you need these services call us first.
You will need to purchase prescription drugs separately.
We recommend choosing the statewide $4 per month generic drug program from Walmart, Target or other pharmacies honoring that program.
